Dr David C. Marsh, MD, CCSAM
Dr. Marsh graduated in Medicine from Memorial University of Newfoundland following prior training in neuroscience and pharmacology. In January 2004, Dr. Marsh began serving as the Physician Leader, Addiction Medicine with Vancouver Coastal Health and Providence Health Care. In this role he is also Medical Director for Addiction Services, HIV/AIDS Services and Aboriginal Health for Vancouver Community. Dr. Marsh is also Clinical Associate Professor in the Department of Health Care and Epidemiology, Faculty of Medicine at the University of British Columbia. Prior to relocating to Vancouver he was the Clinical Director, Addiction Medicine at the Centre for Addiction and Mental Health in Toronto.
Dr. Marsh's research interests include the integration of pharmacotherapy and psychotherapy in the treatment of substance use disorders and focus primarily on novel interventions for opioid dependence. He is presently involved in several research projects including the North American Opiate Medication Initiative (NAOMI) trial of prescription heroin, an Interdisciplinary Health Research Team on Illicit Opiate Dependence in Canada funded by the Canadian Institutes of Health Research and an evaluation of the Supervised Injection Site in Vancouver.
Dr. Bill Campbell, BSc., MSc., MD, CCFP, CASAM, CCSAM, FASAM
Dr. Campbell was born and raised in Alberta. He obtained his MSc from the University of Alberta (Calgary) and his MD from the University of Alberta. Following a rotating internship at Holy Cross Hospital in Calgary he completed two years training in internal medicine at the Foothills Hospital. He began a family medicine practice and developed a special interest in addiction medicine in 1980 and became ASAM certified in 1986. He is a founding member of CSAM having held the positions on the board as Secretary 1995-97, Alberta board Member 1997-2000 and President 2000 – 2002. He is Clinical Associate Professor in Family Medicine at the University of Calgary. He has an Addiction Medicine private practice and is a consultant at the Foothills Hospital Addiction Centre and the Calgary non-hospital medical detoxification centre, as well as being a part time medical examiner. He has one son who teaches in Japan. His interests include fellowship with friends, family and pets, skiing, inline skating, non-linear dynamics and the philosophy of mind and mental health.
Dr. Frank Evans
Dr. Frank Evans acquired his Medical Doctorate and Honours Bsc. in Pharmacology/Physiology at the University of Western Ontario in 1981. In 1978, he joined the Canadian Armed Forces where he became a Flight Surgeon and a board certified specialist in Aerospace Medicine.
In 1992, Dr. Evans did a fellowship in Addiction Medicine, at Loma Linda Medical University, California. In 1993, he acquired his board certification in addiction medicine, with the American Society of Addiction Medicine.
Dr. Evans works full time as an addiction specialist. He is on staff at the Chemical Dependency Detox/Treatment unit, at Humber River Regional Hospital and at Bellwood Health Services treatment facility, where he is director of Bellwood’s Health Professional Treatment Program.
Dr. Evans also serves as chair of the Ontario Medical Association’s, section of Addiction Medicine, and secretary/executive board member for the Canadian Society of Addiction Medicine.

Authority, Fredericton, NB. 50% clinical practice as detox physician,
medical director of methadone program, client review and consultation and member
of integrated team for the management of concurrent disorders; and 50% education,
and program and policy development.
Dr. Suzanne Brissette
Dr. Suzanne Brissette is a general practitioner who started
working in the field of Addiction Medicine in 1989 after 3 years in the field
of Emergency Medicine. She was Medical Chief of the Saint-Luc-CHUM Addiction
Service from 1994-2002. She is Vice-President of the Board of Physicians, Dentists
and Pharmacists of the Montreal University Hospital Centre. She has served
as the Quebec Board Member of CSAM from 1994-2001 and rejoined the board in
2002. Dr Brissette is Assistant Clinical Professor at the Department of Family
Medicine of the University of Montreal. She obtained her Masters degree in
Clinical Epidemiology in 2001. Her research interests include epidemiology
of illicit opioid use and alternative substitution treatment for opioid addiction
and epidemiology, prevention and treatment of hepatitis B & C. She is currently
responsible for the NAOMI clinical trial in Montreal.
Dr. Jeff Daiter CSAM, CASAM, FASAM
Dr. Daiter graduated from the University of Western Ontario in 1991 and completed his Residency in Family and Community Medicine at the University of Toronto. Shortly thereafter, he began his carrer in Addiction Medicine, achieving Certification by both the Canadian and American Societies of Addiction Medicine. He currently acts as the Chief Medical Director for the Ontario Addiction Treatment Centres, a group of 25 Methadone clinics in the province. In partnership with more than 40 other physicians, the Ontario Addiction Treatment Centres offer addiction treatment for over 5,000 patients. Recently, Dr. Daiter was also appointed to the National Buprenorphine Advisory Board and in that role, assisted in developing the National Buprenorphine Core Training Program. In 2004, he was appointed a ‘member at large’ of the Executive of the OMA Section of Addiction Medicine. In 2006 he became a Fellow of the American Board of Addiction Medicine. He is acting Chair for CSAM 2007 Scientific meeting in Ottawa as well as Chair for CSAM’s Standards Committee. He remains committed to offering comprehensive addiction treatment to those patients suffering from Substance Dependence through the province and across the country.
In addition, Dr. Daiter is a Diplomate of the American Board of Sleep Medicine, a Fellow of the American Sleep Disorders Association and Director of the York Region sleep Disorders Centre. His research interest is in the combined fields of Addiction and Sleep Disorders Medicine.
Dr. Kumar Gupta
Dr.Gupta is Canadian certified in Addicton Medicine,and has been practicing Addiction Medicine since !997,after completing a one year Clinical Fellowship in Addiction Medicine at CAMH.He as well practices Emergency Medicine at the Humber Regional Hospital.He has served as a Methadone Peer assessor for the CPSO Methadone program since 1998,and is aswell a Peer Assessor for the Quality Assurance program at the College.He is active in improving the quality of education for Methadone Prescribers,and is a member of the CPSO Methadone Educational Committee. He has recently been nominated to serve on the Methadone Governance Commitee at the College.His interest lie in Patient Advocacy,writing ,and film.His current areas of interest at CSAM are membership promotion,and the Canadian Addiction Medicine Bulletin.
Dr. Charl Els
Dr. Charl Els was born and raised in South Africa. He graduated from
the University of the Free State’s Medical School in 1990, with a distinction
in psychiatry, and completed the Residency Program in Psychiatry and graduated
(cum laude) in 1996. He also completed the South African Psychiatry Boards,
and he is a Fellow of the College of Medicine of South Africa, and practiced
psychiatry for a number of years before he immigrated to Canada in 1999. Dr.
Els completed two clinical fellowships (Addiction Medicine, and Schizophrenia & Tobacco
Dependence) at the University of Toronto and the Center for Addiction and Mental
Health (CAMH) in Toronto under the supervision of Drs. Peter Selby and Robert
Zipurski. He completed the American Society of Addiction Medicine (ASAM) certification
course in addiction medicine is also certified as a Medical Review Officer.
He is active in establishing an integrated treatment model in concurrent disorders
in Alberta, and has been appointed as the President-Elect of the Addiction
Medicine Section of the Alberta Medical Association. He is also the Alberta
director of Physicians for a Smoke-free Canada, and represents this organization
in the Coalition for a Smoke-Free Alberta. His most recent appointment is as
an affiliate of the Ontario Tobacco Research Unit. Dr. Els has received grant
funding from the Alberta Medical Association and Alberta Health and Wellness,
and more recently he received a federal Health Canada grant for a large-scale
project to establish integrated tobacco dependence services in the region.
He is an avid scuba diver and is an enthusiastic beginner-level snowboarder.
Dr. Jennifer Melamed
Dr. Melamed is a family physician in Richmond, BC. She graduated from
the University of the Witwatersrand in Johannesburg, South Africa in 1984 and
obtained a medical Bachelor of Science degree in 1981. After completing
her undergraduate training and internship in South Africa, she immigrated to
Canada in 1987 and worked in a rural GP setting in Nipawin Saskatchewan from
1987 to 1991. during this time she worked as a full service GP and did
obstetrics and emergency medicine as a member of a 6 physician group practice.
She moved to British Columbia in 1991 and began working in a group family
practice. In 2000, she began working in Addiction Medicine at the Vancouver
Methadone clinic. In addition, Dr. Melamed is employed by the Vancouver
coastal Health at a tertiary and quaternary level Addiction Care Centre. She
is a member of the Canadian and International Societies of Addiction Medicine
and is preparing to take the American Society of Addiction Medicine exam in
2006.
Dr. Melamed is involved as a volunteer in the greater Vancouver community
at the Union Gospel Mission and with a organization providing help to the women
of the Vancouver downtown East side.
Dr. Michael Varenbut,
MD, CCFP, FCFP, DABSM, CASAM, CCSAM, MRO, FASAM
Dr. Michael Varenbut graduated medicine with honours
from the University of Toronto Medical School in 1991, and completed a residency
in Family and Community Medicine at Sunnybrook Health Sciences Centre.
After several years in family and emergency practice,
he began to focus his efforts on addiction medicine. For the past
10 years, he has spent a large proportion of his clinical and professional
time in Addiction Medicine, and has also successfully achieved specialty
certification from both the American and Canadian Societies of Addiction
Medicine, and as a Medical Review Officer.
As the Executive Clinical Director of the Ontario Addiction
Treatment Centres (OATC), Dr. Varenbut has promoted the expansion of addiction
services to many under-serviced communities in Ontario. The OATC currently provides
addiction services in over 20 communities, and to over 3000 patients on a
daily basis. As the Clinical Director, he has been responsible for
such aspects as treatment guidelines, policies & procedures, staff and
physician training and education, and liaison with community agencies.
As an active member of CSAM, Dr. Varenbut has promoted
CSAM ’s
mission and has been able to achieve a significant increase in membership. He
has also worked hard to promote and elevate the status of addiction medicine
amongst the medical profession, and improve access to Addiction CME, Training
and physician support.
In addition Dr. Varenbut has also achieved certification in Sleep
Medicine, and is a Diplomat and Fellow of the American Board of Sleep Medicine. With
this unique certification, it has allowed him to treat patients who are afflicted
with the co-morbidity of addiction and sleep disorders.
Dr. Varenbut has also had a significant interest in
primary clinical research in such areas as: MMTP, Effects of Opiates on sleep
architecture, Sleep Apnea syndromes, Hepatitis C treatment in MMTP and Urine
toxicology. Several
of these areas have yielded research papers, publications and text authorships.
Dr. Varenbut has also been committed to education at
multiple levels, which have included undergraduate, graduate and postgraduate
levels through his academic appointment as assistant professor, Faculty of
Medicine, at the University of Toronto, in addition a variety of educational
activities targeted to the public, community agencies, medical and Para-medical
groups.
With the opportunity to act as a board member on CSAM’s board
of directors, he sincerely hopes to be able to further promote the field
of Addiction Medicine, and serve the interests of its members to the best
of his abilities.
Dr.
Robert Cooper, LLB, MD, CCFP, CCSAM
In Canada CSAM plays the essential role of advocate for physicians in the
addiction field and for patients who suffer from addiction disorders. Since
1997, my practice has focused on patients with addiction. As a member of
the board I will assist the Society as it pursues its mission to foster
research, education, and the development of new prevention and treatment
programs.
Current Practice Appointments include: Dufferin-St. Clair Community Clinic
Addiction Treatment and the Humber River Regional Hospital – Department
of Family Practice and Emergency Medicine in Toronto, Ontario, since 1997.
Past Practice Appointments include: Relate to Medicine - Courtesy
staff in Emergency Departments at Etobicoke General Hospital, Toronto East
General Hospital, Humber Memorial Hospital and York-Finch Hospital; Related
to Law – Articling at Goodman & Goodman, in Toronto from 1986 – 1987
and Night Court Prosecutor at Old City Hall Toronto from 1984 – 1987.
